A very good Performance Review Template should have the following:
1) Specific goal
Specify why this is evaluation is needed. Those who are new to the company or the team may not know the value of having a monthly or quarterly or yearly evaluation. Tell them why this is important to the company so that they would know that this is not to just make them feel bad about themselves.
2) Specific rules
Specify how the point system works to the evaluator so that they would be able to score the employee's performance without any confusion.
If there are any other suggestions for the employee, leave a space wherein the evaluators can write it.
