o One of the most important things that you need to include in your template is the objectives. This will make it clear to everybody concern why the review is being done, especially to the employees. Some employees might feel a bit nervous but you have to let them know that this is for their own-and your company's-good.
o It is equally important to include the directions. Clearly state how the evaluators should rate the variables in the template. Should they check specific boxes or should they rate each variable from 1 to 5, 1 as the highest and 5 as the lowest?
o Finally, include several quantifiable variables that can measure a person's performance such as leadership skills, communication skills, time management, and so on.
